Acknowledgement, Waiver and Release from Liability And Approval of Use of Name and Photographs
I acknowledge that my child may participate in dance, tap, jazz, ballet, including warm-up, exercise, training, instruction, attendance at onsite/offsite performances and recitals, which are dangerous physical exercise and comes with the potential for minor and serious injury. On behalf of my registering child, hereby assume the risks of participation in dance, tap, jazz, ballet exercises, warm-up, training, instruction, performance events. I approve that my registering child’s name and photograph may be used by Betsy Daily School of Performing Arts, Inc. to promote its business. I certify that my registering child is physically fit, has sufficiently prepared for participation in dance, tap, jazz and ballet exercises, warm-up, training, instructions, and performance events and has not been advised otherwise by a qualified medical person. I acknowledge that my statements herein are being accepted by the Betsy Daily School of Performing Arts, Inc and relied upon by any and all sponsors, employees, instructors, organizers and administrators in allowing my registered child to participate in dance, tap, jazz, ballet, exercises, warm-up, training, instruction, performance events including recitals. In consideration, for allowing my registered child to participate in dance, tap, jazz, ballet, exercises, warm-up, training, instruction, performance events including recital, I hereby take the following action on behalf of my registered child, child’s executors, administrators, heirs, next of kin and successors and aver a) WAIVE, RELEASE AND DISCHARGE from any and all claims or liabilities for personal injury, property damage or damages of any kind which arise out of or relate to the registered child’s participation on, or traveling to/from for the performance event including recitals, dance, tap, jazz, ballet, exercises, warm-up, training or instruction THE FOLLOWING PERSONS OR ENTITIES: The Betsy Daily School of Performing Arts, Inc., its directors and officers, all event sponsors, instructors, employees, organizers, representatives and agents of any of the aforementioned b) AGREE NOT TO SUE any of the aforementioned persons or entities for any claims or liabilities that I have waived c) APPROVE THE USE OF names and photographs by the Betsy Daily School of Performing Arts, Inc. to promote its business d) HEREBY AFFIRM that I am eighteen years of age older and am acting on behalf of my registered child e) HEREBY AFFIRM that I have read Acknowledgement, Waiver and Release from Liability and Approval of Use of Name and Photographs and understand its contents
